Embrace Nitro Server Handlers

Would be nice to expose helpers for the Nitro Server Handlers.

I think we could have:

addServerHandler()
addServerDevHandler()
addServerErrorHandler()

This way we could also deprecate addServerMiddleware & serverMiddleware and simplify this part of the framework.

@wobsoriano Yes! Runtime config is probably the best replacement you can access it from server with import { useRuntimeConfig } from '#imports'
