Start in Disabled mode

Environment:
- VSCode Version: 1.17.2
- VsCodeVim Version: 0.12.2
- OS: Windows
I wish it was possible to start in disabled mode.
For exemple by adding a parameter vim.startDisabled
false by default.

settings.json
:The cursor might show as vim or as normal and vice versa but it does work as intended. I use CTRL + SHIFT + P to enable vim if I need it.
Why not support it?
